NOTE PRIOR TO STORAGE

Xytex requires recent blood test results prior to storage. If using a Cryokit™, the nearest medical lab can obtain blood work; necessary instructions and paper work are included in the Cryokit™.

Embryo and Oocyte

Xytex requires blood-testing results for both female and spouse, or semen and oocyte (egg) donors, before embryos can be accepted for storage.

We require testing for HIV, Hepatitis B, Hepatitis C, HTLV l and ll (Human T-lymphotropic Virus), and RPR (Rapid Plasma Reagin). Federal regulations may require additional testing at a later date. If a gamete provided for the formation of this embryo is a directed (known) donor, additional tests are required.

We will provide the physician and patient with the following items:

  • Embryo Transfer Tank
  • Service Agreement

Transfer tanks provided by Xytex are specially designed to hold liquid nitrogen and maintain embryos at a constant temperature of -196 Celsius. Fees include annual storage fees and transportation fees from the physician’s office to our facility.

Embryos may be stored indefinitely when frozen in liquid nitrogen – a very cold but non-toxic substance. We utilize the latest technology and recommended procedures to cryopreserve cells and tissue – making them readily available for use when needed.

FACING SERIOUS ILLNESS

Many medical treatments, such as those used in treating cancer, can render a patient temporarily or permanently sterile.

SURGERY

A vasectomy is typically considered permanent and reversals are not guaranteed. Storing semen prior to a vasectomy is a way to make sure it is available in the future, if needed.

SERVICE MEMBER

If an individual’s daily work puts him in danger, including military or first responders, semen storage can be a safe option.

REGULARLY IN CONTACT WITH HARMFUL ENVIRONMENTAL FACTORS

Men who are in daily contact with insecticides, radiation, fungicides, herbicides or heavy metals may want to consider storing semen.

It is important to plan for lead time prior to treatment or surgery.

Directed Donor

A directed (known) donor is a donor that is able to select someone they know to donate semen for insemination.

STEP 1

OBTAIN AN ORDER FROM INSEMINATING PHYSICIAN

You will receive a physician order form that will need to be completed by your health care provider and returned by fax or email.

STEP 2

PROGRAM ACKNOWLEDGEMENT

You will receive a designation of recipient and financial responsibility form and acknowledgement form that will need to be completed and returned by fax or email.

STEP 3

ACCOUNT SETUP ($450 NONREFUNDABLE, INITIAL FEE)

Directed donors are screened for infectious diseases and medical history is reviewed. During this process, both the donor and recipient will receive an authorization to release form or transfer samples (if not storing) and medical history form that will need to be completed and returned by fax or email.

STEP 4

DIRECTED DONOR SAMPLE COLLECTION AND TESTING

Your first visit includes a semen analysis to confirm a sufficient number of total motile sperm.  Blood and urine must be collected for infectious disease testing within 7 days (before or after) of collection of semen samples.  For example, you could collect semen samples for two weeks, as long as blood and urine is collected in the middle of the two-week period.

Testing includes:

  • Chlamydia
  • Gonorrhea
  • HIV-1/2/Oantibodies
  • HIV/HCV/HBV NAT
  • Hepatitis B virus surface antigen
  • Hepatitis B virus core antibodies
  • Hepatitis C virus antibodies
  • Human T-cell lymphotropic virus (HTLV-I/II) antibodies
  • Cytomegalovirus (CMV) total antibodies
  • Cytomegalovirus (CMV) IgG and IgM antibodies (if the total antibodies test is positive)
  • Syphilis (treponemal antibodies)
  • Complete blood count (CBC)
  • ABO blood type
  • Rh (Rhesus factor)
  • Genetic testing is available, upon request, at an additional cost

STEP 5

SAMPLES, BLOOD TESTS AND PHYSICAL

Samples must be left within 7 days of, before or after, blood/urine test and physical exam.

STEP 6

TEST RESULTS AND SAMPLE RELEASE

Once lab results are reviewed and we receive the donor’s signed authorization to release units, units can be released when needed.

Using a Cryokit™

A Cryokit™ is available for patients who do not live near a Xytex facility. The Cryokit™ is an in-home specimen collection kit that allows a patient to collect his semen and securely return the collection to Xytex.

A Cryokit™ is $620. This price includes the Cryokit™, shipping, administration fee, processing of one sample and one year of storage. Processing of additional samples is $75 per sample.

Xytex requires payment at time of service.
