Thank you for inquiring about the Xytex Tissue Storage Directed Donor
program. This service provides you with the means to choose someone you know
to donate sperm for insemination. The following information describes the
steps for fulfilling the Directed Donor Program. XTS realizes how important
this decision is for you and your donor and especially your future child. We
advise you to seek legal counsel once you have decided to pursue this
option; therefore, enabling you and your donor to mutually understand the
intentions and future wishes you will have for your healthy baby.
XTS also recommends that your donor have a semen analysis prior to
proceeding with this program. That way you will both know if he has sperm
cells for donation.
The program is thorough and entails considerable time on the part of all
those involved. We at XTS wish to make this a positive experience for you
and your donor, therefore we have written the following to help answer your
questions concerning this program.
Client Acknowledgement of Xytex Tissue Storage (XTS) Directed Donor Program
Before proceeding with the program, an XTS representative will need to
advise you of certain requirements and other information. Please print a
copy of the Client Acknowledgement form for reference, and call us at
800-277-3210. After speaking with a representative, please sign and return
the Client Acknowledgement form by fax to 706-736-9720.
Medical History Questionnaire (MHQ):
Once you have set up an account with XTS, the MHQ can either be emailed or mailed to your donor. When the MHQ has been filled out, your donor will mail it to XTS for the Medical Director to review. Upon approval of the MHQ by the Medical Director, you and your donor will be notified of the outcome.
Sample Collection:
If your donor is going to collect in the Atlanta or Augusta office, he will need to set up three appointments for the processing of his samples, blood work and physical exam. Additional storage fees are required when your donor collects more than three samples.
Quarantine Period: Semen will remain in quarantine for 6 months. After this, the donor’s blood will be retested to confirm original blood work is unchanged. Once the results have been received by XTS and approved by the Medical Director, the samples can be shipped to your physician for Assisted Reproductive Techniques (ART).

1. What is a Directed Donor? A Directed Donor is a person who you have asked to donate sperm for your use.
2. Do I need to contact a lawyer? Yes. It is highly recommended that you and your donor seek legal counsel in order to have in writing all intentions of both your donor and yourself as the recipient.
3. What are the required blood tests? The following are the required tests to be performed by either XTS or your
donor physician’s laboratory.
- HIV 1 & 2 antibodies
- HIV/HCV NAT
- Hepatitis B Surface antigen
- Hepatitis B core antibody
- Hepatitis C antibody
- HTLV I/II antibodies
- CMV Total assay
- RPR (Treponema pallidum)
- TP-PA or FTA-ABS (Treponema pallidum)

4. Why do I have to wait 6 months before I can have my donor re-tested? Due to our licensing and policies, XTS has a 6 month quarantine period for safety reasons.
5. Can I sign a waiver in order to eliminate the 6 month quarantine period? No. XTS strives to maintain and provide the highest standard of safety when it comes to you and your baby.
6. What forms of payment do you accept? We accept, VISA, MC, AMERICAN EXPRESS, DISCOVER and we have a payment plan provided through MedicalFinancing.com. Please ask for information on this payment plan.
7. What if my donor is out of town for the second set of blood tests? Your donor will have to have his blood drawn for the second set of tests. Please keep this in mind. If he does not have the tests completed, the units will not be released.
8. Can my donor collect more than three ejaculates? Yes. The storage fee for the first 3 ejaculates is $350. If additional collections are necessary, the storage fee will be increased by $100 for the next 3 samples.
9. My doctor requires that I have washed units of sperm, how does my donor have his sperm washed? Your donor will have to make an appointment to collect his sample in our Atlanta or Augusta office. The lab staff will process the sperm as a washed sample. The cost for each washed ejaculate is $200. Also, your inseminating physician maybe set up to wash your donor’s samples.
